A visit is the first step in acquainting you and your loved ones with The Maples’ caring community. Stop in at our beautiful home to experience the warmth and dedication of our staff. Come interact with residents engaged in our various daily activities and offerings. Even join us in the dining room for lunch if your schedule permits.
Should you decide to join The Maples family, our experienced team will do all we can to ease your transition and make it as comfortable as possible. Application Form :
A complete Resident Application consists of the forms included in this packet in addition to the apartment deposit and a one-time application/assessment fee. Filling out the forms completely will help to move the process along smoothly.
Physical Exam :
A physician-administered physical exam is required within 30 days prior to occupancy. You should submit the completed form to The Maples of Towson a couple of days prior to move-in. The requisite State Physician Assessment Form must be accompanied by a tuberculosis (TB) test or a chest x-ray.
The Maples Assessment :
The Maples assessment ensures that we may best serve each resident’s health, wellness, and social needs. An assessment with one of our licensed nurses can be arranged for you either at your home or ours.
The Residency Agreement :
The move-in process is finalized by signing a Residency Agreement outlining services offered at The Maples of Towson. Agreements are made for a period of one year, although it may be terminated at any time with 30 days written notice.
Required Information :
At move-in you will also be required to provide us with the following documentation for our files. We can make copies of your originals at the home.
- Medicare and Private Insurance Cards
- Living Will *
- Power of Attorney *
- Do Not Resuscitate (DNR) Order *
